This makes a LOT, but is so easy and yummy!
Combine first 7 ingredients in a large crockpot, cover and cook on High for 4 hours or Low for 8 hours (potato should be tender).
Mash mixture until potatoes are coarsely chopped and soup is slightly thickened.
Stir in cream, sour cream, cheese, bacon (I use a package of bacon bits instead of the 8 slices because it's faster and easier), and chives.
You can top each serving with sour cream, bacon, and more cheddar cheese if desired. I like it all mixed into the soup.
