Mix everything in a bowl. Add crushed pork rinds until you're happy with the texture. Form into a loaf. Bake in a pan with sides. Bake at 350 until it's done. About 45 minutes? Add ketchup to top if desired & bake another 15ish minutes.
Alternatively, cook in 250F smoker for ~3 hours until meat internal temp is 155-160F.
Stuffed: Cheese + sausage (eg, pepper jack + apple chicken sausage or gorgonzola + keilbasa)
Bacon wrapped: Form a bacon mat around the meat just before you put it in the smoker/oven.
