Melt butter in the top of a double boiler over medium to high heat. (or a regular pot over low heat). Add flour, salt and pepper. Stir until smooth with a wire whisk. Gradually add milk, stirring constantly. Cover and cook about 10 minutes or until thickened, stirring often. Once thickened, add grated cheese and stir until melted.
Meanwhile, bring water to a roiling boil for the macaroni. Add the salt, then add the macaroni, stirring. Boil for 10 minutes and drain. Rinse with cold water.
Add the cheese sauce to the macaroni. Pour into a greased 9x13 baking dish. Sprinkle with any desired toppings. Bake at 400F. for 20 minutes or until bubbly and brown.
