Place cream cheese in slow-cooker on high. Once it softens mix in soup. Place chicken on top and let it cook 10-12 hours. Cook pasta according to package instructions. With two forks, shred the chicken and serve over pasta.
(I add frozen peas to the chicken mixture while the pasta is boiling. It adds color and rounds out the meal.)
This is great for busy moms. Just let the cream cheese soften as you go about your morning routine. Before you leave for work, add the soup and chicken. I don't even thaw the chicken. I just add it frozen! Another way to add color is using tri-color pasta.
